Sometimes supporting someone at home involves more than the time spent together during a visit.
Service Description
Independent Living Support Plus provides ongoing, personalised support for people who are living independently, while giving families greater reassurance that someone local understands how things are going. It is more than a regular home support visit. Alongside practical support and companionship, I take the time to notice what is happening, identify when something may need attention, think through possible solutions and follow things up where appropriate. This may include: Weekly visits of at least one hour Accompanying someone to appointments and helping them understand and remember information Arranging appointments and communicating with services, with appropriate consent Keeping family informed about relevant observations, changes or concerns Researching services, options and practical solutions Helping organise household matters and put simple systems in place Following up on things that need attention between visits Providing a reliable local point of contact when family cannot be there themselves A detailed written report for family, including observations, suggestions and recommendations. More than a visit The time spent supporting someone does not begin and end when I leave their home. Calls, research, appointment follow-up, communication with family and thinking through practical solutions all form part of the support I provide. Independent Living Support Plus recognises and includes this time, rather than charging only for the hours spent face-to-face. Two levels of support Regular Support — from £250 per month One weekly visit of at least one hour, with ongoing observation, practical support, communication and a detailed family report. Enhanced Support — from £350 per month Two weekly visits of at least one hour, providing more frequent contact and a greater level of ongoing involvement. The appropriate level is agreed following an initial assessment and can be reviewed as circumstances change. Visit length and what happens during each visit are shaped around the individual, and I use my judgement to respond to what is needed on the day. The aim is not to take over, but to provide the right level of support to help someone continue living independently and to give their family greater peace of mind. Independent Living Support Plus can also work alongside family members, care providers and other professionals where additional support is needed.
